Comment on POA - Power of Attorney for your Demat account

In POA Form,

what are the things i need to fill ?

Have to enter values for fileds in Witness table ?

What are the mandatory fields ?

If I send POA to you, how did you connect with my account? Because, the form contains only Account holder Name.
